Each note displays his NPI number, state license information, and includes a unique QR verification code so the document can be authenticated. If we're unable to issue your note after review, we provide a full refund.

SickSlip notes include everything an employer or HR department looks for when verifying physician documentation: the physician's printed name, NPI number, state medical license, absence dates, and a QR verification code that anyone can scan to instantly confirm the note's authenticity at sickslip.co/verify. In our experience, employers who require physician documentation for absences accept these notes the same way they'd accept any physician-signed note. If yours doesn't, we'll refund you in full.

We do not provide FMLA paperwork, the Amazon Healthcare Provider Form, disability certifications, extended leave-of-absence documentation, fitness-for-duty / return-to-work clearance, school physicals, or any documentation requiring a treating physician relationship. If your employer is asking for any of those, please see your primary care physician or a telehealth service that explicitly handles FMLA paperwork.

No. The Amazon Healthcare Provider Form is part of Amazon's FMLA / Leave of Absence process and requires a treating physician relationship and a formal Serious Health Condition certification under federal FMLA law (29 CFR 825.306). The U.S. Department of Labor specifically requires a video visit (not asynchronous review) to satisfy FMLA's clinical evaluation standard. SickSlip is an asynchronous service for routine short-term absence notes only — we are not able to issue FMLA documentation, complete the Amazon Healthcare Provider Form, or modify an existing note to add diagnosis or symptom information. If your Amazon HR team is requesting any of those, you'll need to work with your primary care physician or a telehealth service that offers scheduled video FMLA visits.
