These policies apply to services provided by SickSlip, Inc. (technology and administrative services) and SickSlip P.A. (clinical review and medical documentation). These policies supplement and are subject to our Terms of Service.

Eligibility
- You must be located in a state where our physician holds an active medical license.
- SickSlip covers short-term, non-emergency illnesses only — common colds, flu, stomach bugs, migraines, back pain, respiratory infections, and similar conditions.
- We do not provide notes for: extended medical leave (FMLA/disability), legal proceedings, medical clearance, personal time off, or conditions requiring ongoing specialist care.
- Maximum absence coverage per note: 5 consecutive days.
- Maximum notes per patient: 3 notes per account.
Note Modifications
After your note has been approved and delivered, you may request a modification under the following conditions:
- Modification requests must be submitted within 72 hours of purchase.
- Requested dates must fall within or adjacent to the original absence period. We cannot issue notes for dates significantly different from your original request.
- The total absence period cannot exceed 5 consecutive days.
- Modifications are reviewed by the physician and are subject to clinical discretion.
- There is no additional charge for approved modifications.
Modification requests that fall outside these guidelines will be denied. If your request is denied, you will receive an email explaining the reason.
Refund Policy
- 100% money-back guarantee if your employer or school does not accept your note.
- Refund requests must be submitted within 30 days of purchase.
- To request a refund, email support@sickslip.co with your order details and a brief explanation.
- Refunds for rejected notes are processed automatically when the physician denies the request.
- We do not offer refunds for buyer's remorse, duplicate purchases made in error (contact support for resolution), or notes that were accepted by the employer/school.
Pricing
- Standard note: $29.99 — delivered same day.
- Rush note: $37.99 ($29.99 + $8 rush fee) — delivered in under 10 minutes.
- No subscriptions, no hidden fees, no follow-up charges.
- Promo codes, when available, are applied at checkout.
Account & Access
- After your note is approved, you will receive an email with a link to create your account and download your note.
- Your signed PDF is available for download from your dashboard at any time.
- If you are locked out of your account, contact support@sickslip.co and we will send a password reset link.
- Account data is retained for 90 days after your last note, then permanently deleted in accordance with our data retention practices. Medical records required by law may be retained longer as required by applicable state and federal regulations.
Verification
- Every note includes a unique Document ID and QR code.
- Employers and schools can verify note authenticity at sickslip.co/verify or by scanning the QR code.
- Verification confirms the document was issued by a licensed physician but does not disclose any patient health information.
Fraudulent Use
SickSlip reserves the right to deny service, refuse modifications, and withhold refunds if we determine in good faith that a request is fraudulent, involves fabricated or exaggerated symptoms, or is intended to deceive an employer, educational institution, or any other party. Accounts associated with fraudulent activity may be permanently suspended.
